An insurance policy is not a guarantee of coverage for services. It is the patient's responsibility to communicate with your insurance company to determine your coverage/copay/deductible.

 

**NOTE: Blue Cross Blue Shield patients - if your policy begins with prefix BEG or PGX, you must contact your Primary Care Physician  to obtain a referral prior to your appointment date. If we do not receive a referral by the date of your appointment, BCBS will not cover the visit and the patient will be responsible for the bill.
